For a medical office, for example, an employee might think, “I’ll ask ChatGPT to help rewrite this letter to a patient,” and paste in the full note — name, date of birth, diagnosis and all. Even if the intent is good, the data handling is not. At an absolute minimum, that information must be fully redacted before it ever touches a public AI tool, and even then, you should ask whether it needs to leave your environment at all.

On top of that, AI systems can still produce hallucinations — made-up facts, citations, or references that sound confident but aren’t true. If nobody checks the output, those hallucinations can show up in:

When I ask executives a different question —

“Where, in your core operations, does AI take something from request → action without human copy/paste in the middle, and how are we governing its use?”

— things often go quiet.

Step 1: Pick one painful workflow

Look for something that is:

  • Repetitive
  • High-volume
  • Annoying to everyone involved

Examples we see a lot:

  • “Where’s my [order/claim/case]?” inquiries
  • Basic IT issues (passwords, access, simple setup problems)
  • Early-stage intake and triage

Step 2: Sketch what a SMART AI Agent would actually do

On one page, answer:

  • What information does it need to see?
  • What decisions can it make safely?
